Healthcare systems across the world are facing an augmenting burden of chronic disease. The need for better integration of care systems and patient pathways has been increasingly recognised in the last decades. The financial organisation and the reimbursement system play a constituent part in the management operations of a healthcare system, together with legal frameworks, clinical guidelines, norms, forums for collaboration, information support systems and care practices. Payment reform is considered an important component of a broader set of strategies to achieve greater degrees of care integration.
