ICIC15 – 25-27 March 2015, Edinburgh, Scotland

The 15th International Conference for Integrated Care: “Complex Needs, Integrated Solutions: Engaging, empowering and enabling people for active and healthy living”, took place at the Sheraton Grand Hotel, Edinburgh, Scotland from 25-27 March 2015.

The conference exchanged knowledge, experience and new ideas in the design and delivery of integrated health and social care. Key themes of the conference included:

• Engagement and co-production with individuals, carers, families, communities, care workers and decision makers
• Tackling multi-morbidities, health inequalities and complexity with Integrated Care
• Transforming care systems: evaluation and continuous improvement; and Digital health and innovations.
